The Georgia
Bankers Association Insurance Trust, Inc. offers a benefit called
Beyond Disease Management. This benefit is offered free to
eligible members to provide additional services and support to help you
understand and direct your medical care.
What Is Beyond
Disease Management?
Beyond
Disease Management is designed to help keep you the healthiest you can
be. It gives you the tools and information you need to manage your
health. This free program is available to members with chronic
illnesses and conditions like Asthma, Diabetes, Heart Disease, etc.
If you are eligible
for this program, nurses can help you and your family understand your
illness and what it means to your lifestyle. They will explain the
options available to you, and will work with you and your doctor to make
sure you understand your care.

Disease Management Do for You?

can provide support for you and your family in managing your health.

It can help
educate you and your family on your health needs.

It can serve as
a link between you and your doctor.

It can help
obtain discounts for care and services from your doctor, hospital
and insurance company.
How Does Beyond
Disease Management Work?
A
nurse will contact you and explain the program to you.
You will be asked to
complete a health questionnaire to determine final eligibility.
Program members receive:

Periodic health assessments
-
Telephonic access to a 24-hour Care Line and Health Information
Library
-
Education and information on diet, exercise and other health topics
-
Contact with a Care Manager to discuss medication and other health
needs or concerns
This program
respects your privacy. Your health information will be kept
confidential, and will only be shared with the people you choose.
How Do I Know If I
Am Eligible for Beyond Disease Management?
You may
be eligible if:

You are an
employee or dependent covered under our health plan
-
You currently
have a chronic or progressive illness
-
You are
currently in Case Management
-
Your insurance
company refers you to the program
-
You may also
refer yourself for the program.
